Due by Monday, October 29, 2018:
1) Completed player packet forms - Partially completed forms will not be accepted.
2) Sports participation fee of $35 - This is a one time fee for the 2018-19 school year.
3) A valid sports physical - Sports physicals must be performed by a licensed medical professional and are valid for 12 months from the date they are completed. (If you played basketball last year and have not had a sports physical since then, you must have one to participate this year.)

-- Parents are responsible for transporting their student to practices and games.* If your student is picked up in car line each day, please plan to drive your child to practices and games. If you can help transport other students, please complete the Carpool Interest form (will be available at the Mandatory Parent Meeting). If your student is a bus rider or GEARS participant and you need assistance with transportation, sign up on the Carpool Interest form to get connected with other parents to take turns carpooling. We need all parents involved to make the season a success! Coordinating carpool schedules will be the responsibility of interested parents. Lead Academy will share contact information among interested families, who can coordinate carpools as needed.

-- Practices and games will usually be held on Monday, Tuesday, & Thursday. We will occasionally have a practice or game on a Wednesday or Friday.

-- Admission is charged at all games. Admission is $3 for adults, $2 for students/children, no charge for children 5 and under, or $5 per Family (Family = parents and siblings of the player).

-- Parent volunteers are needed at each game to: collect admission, keep the game book, run the game clock and provide snacks/drinks. You will get in FREE to games when you volunteer!

If you have additional questions about basketball, please contact Mr. Keller.

* Bus transportation will be available for some games. Parents will be notified when buses will be used to transport players TO games. Parents will be responsible for picking up students FROM all games.
